How do I apply?
If you are aged between 16 and 40 (inclusive), fit our medical criteria and are resident in the UK, to apply for a special day please complete our official official special day application form.
Download the special day application form here, or alternatively call 01707 259777 to request a copy to be sent to you in the post.
You will need to ask a medical professional (e.g. a consultant, GP, nurse, etc ) who knows you and is involved in your care and treatment, to fill out and sign the form. They will also need to be available to speak to the Willow Foundation during office hours to verify your application. All information is treated with the strictest confidence.
You will also need to provide details of a responsible companion - someone aged 18 or over who will accompany you on your special day. They must also sign the form.
Applications must be completed in full and signed by the applicant, their responsible companion and a medical professional. Unfortunately we are unable to process incomplete or out of date application forms and will need to return them to your for revision.
