Do you know someone who deserves a special day?

For those living with a life-threatening illness, every day deserves to be treated as precious. Sadly the stress and difficult realities of diagnosis and treatment mean that it's all too easy to forget that life is here to be lived.

The Willow Foundation is a registered charity that aims to provide psychological and emotional support for seriously ill 16 to 40 year olds through the provision of special day experiences.  At a time of uncertainty, spending quality time with family and loved ones can help restore a sense of normality, bosst confidence and create precisou memories for the future.

We aim to work closely with referring medical professionals to help provide memorable experiences for patients in your care.

Your involvement

Every young adult who applies to the charity for a special day must have their application endorsed by a medical professional involved in their care and treatment. As their referring medical professional, you will be asked to fill out and sign the Professional section on the official special day application form. This section on the form asks for medical information relating to the individual's medical condition.  It is important that we are aware of treatment schedules and hospital/hospice stays so we can ensure we meet their particular needs.    All information will be treated in the strictest confidence. 

Once we have received the completed application form, one of the charity's medical advisors will be in contact with you to verify the information on the form and to discuss arrangements for the special day. Unless there is a particular medical requirement, you will not be asked to go on the special day.

Post special day you will be sent a questionnaire asking you to evaluate the chairty's service.  We would be grateful if you would complete the questionnaire.  All comments will help the chaity to develop its service for the future.
